Centerstone Solutions, an employee assistance program (EAP), is a workplace service focused on helping employees and their household members to function at their best at work and at home. The forms library assists network providers in delivering the highest quality of care with an EAP focus. Required and highly suggested forms can be downloaded from the library to meet the objectives of the EAP.
- Statement of Understanding
Providers are required to review the scope of the EAP with client during first session, obtain signature, and return with opening documents to the EAP.

Complete and return the closing form after last session with client to EAP.
- Provider Billing Form
Services should be billed within 45 days of service date.
- W9 Form
Providers must have a current W-9 on file for payment.
